Terms & Policies
Appointments and Cancellation Policy:
Advanced reservation for our service(s) is recommended.
A major credit card is required to hold your reservation(s).
If you need to cancel or reschedule, we require 24 hours’ notice.
Appointments cancelled, changed or rescheduled in less than 24 hours will be billed 100% of each service and will be charged to the credit card on file.
Rescheduling Day Of:
If you need to reschedule day of, it is considered a cancellation, and you will be charged 100% of the fee.
Your appointments are reserved for you and only you, and attached to the appointed time. We have these policies in order to provide you with the best possible service.
No Show:
If you do not show up to your appointment, the credit card on file will be charged for 100% of the fee.
Age Requirement:
Guests under the age of 18 must have parental consent.
Gift Cards:
Gift Cards are a wonderful way to give the gift of health, wellness and relaxation to the special people in your life. Gift Cards can be purchased online. Gift Cards may only be used at the Orange location. Gift Cards are treated as cash, and all lost or stolen gift cards are not replaceable. Our gift cards never expire for their cash value. All sales are final.
Gratuity:
Gratuities are accepted. If you were happy with your service, it is customary to give 20% of the full menu price in gratuity to your therapist.
Payment Methods:
We accept cash, Visa, MasterCard, American Express, and Discover cards. We do not accept personal checks. Gratuity is accepted in cash only.
HSA/FSA:
We currently accept both HSA and FSA cards at our office. You must call and check the ‘conditions of use’ for your plan. Some plans require a prescription for treatment from your doctor as well as a diagnosis. We are happy to provide medical receipts based on this information, for your insurance.
Health Insurance:
We cannot accept other forms of insurance, but we are happy to provide a medical receipt for you to submit on your own. To find out if you have coverage, call your carrier and ask if you have ‘manual therapy' coverage provide by a 'massage therapist', some plans may have coverage but it requires you to go to a physical therapist or doctor.
Email Communication:
By providing your email address you are consenting to the use of email communication between myself (name of patient) and representatives of Azure Therapeutic Massage. I recognize that there are risks to its use, and despite best efforts, he/she cannot absolutely guarantee confidentiality. I understand and accept those risks and the policies for email use outlined in the form. I further agree to follow these policies and agree that should I fail do so, Azure Therapeutic Massage may cease to allow me to use email to communicate with him/her. I also understand that I may withdraw my consent to communicate via email at any time by notifying Azure Therapeutic Massage in writing or simply by unsubscribing.

Azure Therapeutic Massage reserves the right, in its sole discretion, to revise these Terms of Use at any time and correct any errors or omissions in any portion of the Site and Materials (as such term is defined below). Such changes shall be effective upon publication on the Site. Materials may also be changed, modified, added, or removed or updated without notice at any time; however, Azure Therapeutic Massage is under no obligation to update Materials. Azure Therapeutic Massage may also make changes in the services, products, programs or prices (if any) described in the Site at any time without notice. Your continued use of the Site after any change Azure Therapeutic Massage makes to the Site will be considered acceptance of those changes.
